Jason Boylan, FNP-BC
Since opening Third and Church Healthcare in 2009, Jason has built his business on the delivery of quality, convenient care. Jason brings a breadth of knowledge to the practice, having worked as a nurse practitioner in emergency medicine, internal medicine, primary care, and pediatrics. He holds Master’s of Science in Nursing from Vanderbilt University and a Master’s in Counseling from West Virginia University. Prior to entering medicine, Jason worked as a therapist with at-risk youth and families. Jason lives in Germantown and is happily married to his wife Caitlin, and the proud father of two boys, Jameson and Charlie.
Moses “Andre” Swauncy, MD
Dr. Swauncy is board certified by the American Board of Family Medicine has been the medical supervisor for 3rd and Church Healthcare for several years. Dr. Swauncy does not see patients in this practice, but is available for real-time consultation and helps ensure excellent patient care by performing chart audits.
Laura Couture, FNP-BC
Laura Couture is a Family Nurse Practitioner who specializes in family medicine and aesthetics. She has worked both in primary care and urgent care settings and discovered her passion for aesthetics in 2016. She has completed extensive training through Empire Medical Training and Palette Resources in the use of Botox and Dermal Fillers and has spent the last three years perfecting her craft. She is passionate about making people look and feel their best. Her philosophy is to use aesthetics to enhance her patients natural beauty and restore the inevitable effects of aging. She understands each patients aesthetic goals are unique, and is dedicated to meeting the personalized needs of each patient she encounters. Her favorite part of aesthetics is seeing the joy and confidence that is restored within each of her patients through these simple non surgical treatments. Laura is originally from Michigan and graduated from Michigan State University in 2009 with a Bachelor of Science in Nursing and from Loyola University, Chicago with her Masters of Science in Nursing in 2014. She moved to Nashville in 2015 with her husband and dog Emerson. She loves anything outdoors and is an avid football fan. In her down time, she enjoys running, traveling, and taking her therapy dog, Emerson, on visits to nursing homes and hospitals.
Lisa Reagan, Medical Assistant
Lisa is a a highly skilled medical assistant and is usually the first smiling face you see when you come in. She has several years experience and has worked in multiple settings including Vanderbilt hospital and mental health facilities.
Heather Hoover, X-Ray Technician
Heather comes to our team with a wealth of experience. She is a highly skilled x-ray tech and medical assistant. She has worked in internal medicine, orthopaedics and also extensively in phlebotomy.
